Hospital discharge is a complex process representing a time of significant vulnerability for patients. Improving care transitions is critical to improving individuals' quality of care, quality of life, and their outcomes. Healthworks is pleased to announce that your facility has an opportunity to participate in a project designed to implement and expand an effective discharge planning process.
According to Medicaid.gov, effective care transitions:
- Prevent medical errors;
- Identify issues for early intervention;
- Prevent unnecessary hospitalizations and readmissions;
- Support patient preferences and choices; and
- Avoid duplication of processes and efforts to more effectively utilize resources.
Employee Health, Wellness and Safety Cohort 2023
Building an employee health program for health promotion, disease prevention and safety consciousness, can be challenging in these changing times. Healthworks is pleased to announce your facility has an opportunity to learn from peers; to hear how they have built employee health, wellness and safety programs which promote healthy, productive staff and safe working environments. This cohort project is designed to implement and expand an effective employee health, wellness and safety program.
Efficient Revenue Cycle Processes
Reviewing internal processes and creating efficiencies is a recommended strategy to prepare for value based purchasing. KRHOP is pleased to announce an opportunity for Critical Access Hospitals to participate in a project designed to improve the efficiency of revenue cycle processes with the application of Lean techniques and tools.
Transportation Issues in Rural Kansas
In 2020, the Kansas Office of Primary Care and Rural Health at the Kansas Department of Health and Environment and Healthworks launched a project to learn more about community perspectives about the impact of transportation issues on the health of Kansans. Key partners included the Kansas Hospital Association, Wichita State University Community Engagement Institute and the Kansas Health Institute.
