Hospital discharge is a complex process representing a time of significant vulnerability for patients. Improving care transitions is critical to improving individuals’ quality of care, quality of life, and their outcomes. Healthworks is pleased to announce that your facility has an opportunity to participate in a project designed to implement and expand an effective discharge planning process.
According to Medicaid.gov, effective care transitions:
- Prevent medical errors;
- Identify issues for early intervention;
- Prevent unnecessary hospitalizations and readmissions;
- Support patient preferences and choices; and
- Avoid duplication of processes and efforts to more effectively utilize resources.
Hospitals participating in this project will assess their existing care transitions deficits and identify priorities for improvement. Resources/guidelines will be available to strengthen an existing program as well as provide a roadmap to enhance the discharge planning process.
The entire project will use a virtual format and kicks off on November 10. Participation includes access to technical experts in care transitions and readmissions, project management staff to help identify opportunities for improvement and a platform for you to share/brainstorm with peers. Additionally, you will be provided with tools and resources that can be applied for continued improvement after the conclusion of the project.
We are looking for 6 to 9 hospitals to be part of this project. While resources are at a premium, there is no better time to be involved and create stronger discharge planning processes and reduce readmissions in Kansas Critical Access Hospitals.
